Missed Appointment Agreement Missed Appointment Agreement Kindly Give 24 Hours Notice Thank you for choosing Chicago Animal Rehab (C.A.R.E.) for your pet’s rehabilitation care. In order to provide the highest quality medical care to our patients, C.A.R.E. has a Missed Appointment Policy. Please review the following agreement and sign at the signature line, indicating that you understand our policy. I understand and agree with the following: I am responsible for canceling appointments within 24 hours prior to the appointment. Should I fail to attend my appointment or cancel my appointment within the 24 hour period prior to my appointment, C.A.R.E. will notify me of the missed appointment. I will be charged a missed appointment charge of $60.00 for each appointment missed after the first missed appointment. Underwater treadmill package holders will forfeit the session. C.A.R.E. may terminate my services due to noncompliance if I have too many missed appointments.
